They work with you one-on-one to help coordinate you or your child’s healthcare needs. Our Care Management team is trained to help you and your family with any special medical problems like asthma or diabetes, as well as other medical conditions.
We can also work with you if you need help figuring our whether you or your child needs medical care from a provider, an urgent care center or the emergency room.
You may hear from our team if:
- Your primary care or healthcare provider contacts us.
- You or your guardian or caregiver asks us to contact you.
- We think we can help based on your medical condition.
Success Story: Empowering a Young Patient
A HealthVine member struggled to manage their new medication due to a medical condition –poor medication management put them at higher risk for having an emergency room visit.
A HealthVine Community Health Worker (CHW) connected with the child and their family and began making weekly home visits. During these visits, the CHW worked with the child to develop a system to take their medication more reliably. After two months, the member takes their medication without assistance and feels more empowered by their care team to be more independent.

How to Get Connected to CincyKids Connect Services
When your child needs to see a pediatric expert right away, CincyKids Health Connect is here for you with video visits from your smartphone or computer. It’s a great option for those times when your primary care provider isn’t available, and you need a quick answer.
You don’t need an appointment. You don’t even need to leave your house. Video visits have a short wait time, are FREE for HealthVine members, and you’ll get the great care you know and expect from Cincinnati Children’s.
Not sure if you are a HealthVine member?
Your first stop should always be your primary care physician as your place for regular care and they may also have video visits available. In some cases, your primary care provider may not be available, and when that happens, we’re here for you.
Video visits address minor injuries and illnesses like coughs, colds, rashes, and cuts/skin injuries. If medically appropriate, we can send a prescription to your pharmacy. A consultation is not a guarantee of a prescription. CincyKids Health Connect can see HealthVine patients under the age of 18.
Video visits are available from 6 am to 10 pm. They are not the same as in-person evaluations and are not meant for life-threatening situations. For these types of conditions, you need an in-person evaluation at Cincinnati Children’s emergency room locations.
CincyKids Connect: FREE Care When and Where You Need It
With just a few clicks, you and your child can meet with one of our pediatric experts for a video visit. They can review your child’s condition, provide guidance to help you move forward, or if needed, send a prescription to your pharmacy.
Here’s how to get started:
- Download the CincyKids Health Connect app on the App Store or Google Play.OR visit us the CincyKids Health Connect website.
- Set up your account — it takes just a few minutes.
- Fill out a brief medical history, just like you would at your healthcare provider’s office.
- Connect with a pediatric expert.
