HealthVine Care Management
The HealthVine Care Management team consists of registered nurses, social workers, community health workers and other outreach staff. They work with you one-on-one to help coordinate you or your child’s healthcare needs.
HealthVine Care Management Can:
- Connect your child to a healthcare provider
- Schedule appointments
- Arrange transportation to healthcare appointments
- Help fill prescriptions
- Remove barriers preventing your child from getting the care they need
- Connect you with community resources
Youth are eligible for HealthVine Care Management if they:
- Are covered by CareSource Medicaid
- Live in Adams, Brown, Butler, Clermont, Clinton, Hamilton, Highland, or Warren county.
- Are in one of the following age groups:
- Newborn until their 19th birthday, or
- Newborn until their 21st birthday and is in either the Aged, Blind, or Disabled; Waiver; or the Adopted and Foster Care program.

HealthVine OhioRISE
HealthVine OhioRISE provides care coordination services for members with complex behavioral health needs through the OhioRISE program.
HealthVine OhioRISE Can:
- Provide your family with a wraparound approach to managing your child’s illness
- Convene your Child and Family Team (CFT) to identify goals focused on your voice and choice
- Bring your family’s informal and formal supports together to work with your therapist, teacher, pediatrician, and community partners
- Create links to services and supports
- Support transitions of care with change in care settings, graduating from the program, or aging out of services
- Formulate effective crisis and safety plans to address your child’s needs
Youth are eligible for HealthVine OhioRISE if they:
- Are eligible for Ohio Medicaid
- Live in Eastern Hamilton, Clermont, Brown, Adams, Scioto or Lawrence county
- Have significant behavioral health needs, as defined by:
- An assessment with Ohio Child and Adolescent Needs and Strengths (CANS) or
- A recent history of requiring residential or inpatient mental health or substance abuse treatment
- Are in the following age group:
- Newborn until their 21st birthday*
*For youth enrolled in the OhioRISE 1915 (c) Waiver, services will continue until their 22nd birthday.
