What is the best way to refer a youth to HealthVine for care management or care coordination services?

If interested in care management for AmeriHealth, Anthem, CareSource, or Humana Medicaid-covered youth, submit referrals to our HealthVine Care Management team by calling 855-422-8894, option 1 or by emailing HealthVine@cchmc.org. If interested in OhioRISE care coordination services, call 513-636-5007 or email OhioRISE@cchmc.org to learn more about our services.

Does HealthVine attend community events?

The HealthVine team welcomes and seeks out opportunities to partner with community organizations to support events and other initiatives. To inquire about HealthVine attending an upcoming event, please email the Community Engagement team at HealthVineCE@cchmc.org.
