Two Ways to Find a Provider Near You
Contact the Cincinnati Children’s HealthVine Care Management team at 855-422-8894, option 1 or email HealthVine@cchmc.org.
You can also find an in-network provider via CareSource Member Services. Use the CareSource Find a Doctor tool. Does your child currently see a provider different than the provider listed on their insurance card? Complete and send in a PCP Change Form or contact CareSource Member Services to update the information about your child’s provider.
