Primary care practices and behavioral health organizations have specialized expertise in the health of the children and families they serve and a deep understanding of their unique needs. At Cincinnati Children’s HealthVine, we’re here to support, empower and amplify the amazing work these providers do in communities across Southwest Ohio.

HealthVine provides your primary care practices and behavioral health organizations with resources, tools, data and analytics that help you focus on elevating the care you provide patients.

How HealthVine Can Support Your Organization

One of our primary responsibilities is to support the coordination of care for children and youth. The HealthVine Care Management team of care managers, social workers, outreach coordinators and community health workers are available to assist with:

  • Coordination of care
  • Patient needs assessments
  • Depression and anxiety screening
  • Health education and much more!

HealthVine’s care management team has specialized programs to support specific populations and children and youth with care needs including:

  • Transitions of care
  • Chronic and complex conditions
  • Frequent emergency department utilization
  • Newborn and maternal health
  • Children and youth transitioning to adulthood
  • Children and youth in foster care and waiver programs

Success Story: Healing a Patient and a Family

When a young patient was admitted to the hospital, the HealthVine Care Transitions team reached out to their family. While the patient’s recovery was going well, the family faced housing and financial challenges. The HealthVine team stepped in to support and connected the family to additional resources, ultimately helping the family to stabilize their housing situation and focus on their child’s recovery.

For children and youth who need behavioral health care, HealthVine works to ensure they get the right level of care, at the right time and the right location. We strongly believe in the power of an integrated approach to behavioral health care and are working to expand access to behavioral health services in primary care. HealthVine also offers access to behavioral health workforce training opportunities and multilevel, evidence-based care in the community.

Additionally, HealthVine care managers support children with behavioral health needs to help them, and their families, overcome barriers and access appropriate, high-quality services.

Project ECHO

Project ECHO (Extension for Community Care Outcomes) is a guided-practice model that utilizes didactics and case-based learning to help spread evidenced-based behavioral health interventions.

Integrated Behavioral Health

Could your practice benefit from integrated behavioral health supports? Partnering with HealthVine gives providers a unique opportunity to increase their patients’ access to behavioral health care. HealthVine partners with practices interested in integrated behavioral health providers into the primary care setting by providing consultation and expertise in this model, as well as opportunities for formal partnerships to implement the integrated behavioral health model.

OhioRISE is a specialized Medicaid managed care plan for youth with complex behavioral and multisystem needs that launched July 1, 2022.

HealthVine serves as the care management entity (CME) and provides care coordination services for youth in East Hamilton, Clermont, Brown, Adams, Scioto and Lawrence counties. Other CMEs are covering other counties across the state.

Your patient may be eligible for OhioRISE (Resilience through Integrated Systems and Excellence) if they:

  • Are Eligible for Ohio Medicaid
  • Are NOT enrolled in MyCare Ohio
  • Have significant behavioral health needs, as defined by:
    • An assessment with Ohio Child and Adolescent Needs and Strengths (CANS) or
    • A recent history of requiring residential or inpatient mental health or substance abuse treatment
  • Are in the following age group:
    • Newborn until their 21st birthday*

*For youth enrolled in the OhioRISE 1915(c) Waiver, services will continue until their 22nd birthday.

Elevating the care you provide patients takes time and resources. The HealthVine Quality Improvement & Data Analytics team provides practices and organizations with resources and project opportunities to support ideas for innovation and improvement. Using our methodologies, our quality improvement (QI) and data team can help you:

  • Develop measures and set goals
  • Design, test, and implement solutions
  • Analyze data and leverage learnings

Pediatric Improvement Network for Quality Care (PINQ)

The Pediatric Improvement Network for Quality Care (PINQ) is a learning network comprised of primary care sites, caregivers, community-based organization and mental health organizations from multiple sites in the Greater Cincinnati area. The network aims to provide excellent and equitable care in primary care settings through strong partnerships.

Current PINQ project areas of focus include Well Child Care and Mental Health. Future project ideas include Social Determinants of Health (SDOH), chronic conditions and improvement projects for mental health organizations.

Pediatric Population Health Improvement Network (PHIN)

The Pediatric Population Health Improvement Network (PHIN) is a learning network comprised of collaborators from world-renown healthcare systems aimed at developing and sharing population health research and interventions to drive equitable care and improve child health outcomes.

The network’s projects are uniquely focused on population health interventions and addressing health disparities and social determinants for patients and families.

Driving Efficiencies for Youth, Families and Providers

Pediatric expertise is at the heart of our utilization management (UM) process, and our goal is to make it efficient and effective for providers and the young people they work with.

HealthVine works with both behavioral and physical health providers to ensure members get the medically necessary services they need, at the appropriate level, in an appropriate setting, and in a timely and efficient manner.

We’ll be happy to provide the clinical rationale or criteria we use in making medical necessity determinations.

Financial Incentives

HealthVine’s goal is to support practices and organizations in improving outcomes for children across our region. We’re moving toward this goal by providing resources that enhance children’s access to evidence-based treatments while increasing efficiencies in the healthcare system.

All HealthVine Partners are enrolled in our Financial Incentive Program and have the opportunity to earn incentive payments for engaging with our team and achieving clinical milestones*.
