At Cincinnati Children’s HealthVine, we work with community benefit organizations that support the well-being of children and families in Southwest Ohio. After a HealthVine member connected with HealthVine care management services, the team learned the member’s family was facing poor housing conditions.
HealthVine worked with several community organizations to find a new place for the family to live and to support the family with other identified needs.
This collaboration led to a better living situation for the family and helped the member receive the additional medical support they needed.
